These responsibilities continue long after discharge\, shaping long‑term wellbeing\, and influencing health outcomes over time. \nGrounded in transplant and oncology\, this session will highlight why integrating caregivers and supporting their psychosocial needs is a strategic imperative for health systems—improving patient outcomes and strengthening care delivery. The insights shared apply broadly to many serious and chronic conditions that require high levels of care coordination and caregiver support. Together\, we will explore both the clinical importance and the business case for supporting family caregivers as essential members of the care team.
